My CAP proscribing physician has retired, seeking recommendations for AZ, CA or WA preferably but others considered

After 10 months on the Stratton version of the protocol there are definitely signs of improvement, sadly my prescribing physician (rheumatologist) has retired and moved out of state so I need to find a replacement.

Hopefully moving to Phoenix AZ next month so looking for any non-MSi  proscribing Drs. willing to prescribe the Vanderbilt CAP either in Arizona, California or Washington state.

Started on doxycycline 100mg BID and rifampin 300mg OPD
After three months added Azithromycin 250mg m/w/f
after four months started pulsing with Flagyl 500mg TID for five days a month
after six months substituted minocycline for the doxycycline to alleviate intestinal distress
after nine months increased rifampin to 450 mg OPD ultimately to build up to 600 mg

I'm commenting here just to bump the topic and get it noticed. (I truly don't understand how doctors can just retire and abandon their patients. I would expect a referral to a like-minded doctor, or at least a year's worth of abx prescriptions to see me through the time spent searching for a new physician.)
